What should I bring to my first visit?
A photo ID and your insurance card, if you have one. Wear clothes you can move in. No health forms are needed before your visit.
Where do I park?
Parking is behind the building, and there is street parking nearby. Check the posted limits.
How will I hear back?
The front desk calls or texts you by the method you pick, during office hours. This form is not watched around the clock, so for anything urgent, call us.
Can someone come with me?
Yes. A friend or family member is welcome in the room if you would like them there.
